The Countess is among the most sought-after early game farms since she will drop a Rune nearly every killing you do. In fact, during Nightmare, she can drop as many as 6. Runes (though you’ll barely ever see 3 Runes, let alone anymore)! If you can farm her long enough, it allows you to create any Runeword you require to build your character.
This is why The Countess is among the most frequently encountered early-farmed bosses available at the beginning of every Ladder or any single Player who wants to get ready with Runewords to play Nightmare and Hell. Most people do not bother with the junk around The Countess when they complete these runs. They also skip as much garbage as can be snipped out of The Tower dungeon as a complete.
In essence, people race around The Tower as fast as possible, kill the Countess, grab the High Runes she drops, and take off immediately to run again. Two things are worth noting about the Countess… One is that she will always be spawned on the opposite part of the room you walk into.
In the second, based on your map layout, the Countess could be tied into her bedroom. You’ll have the ability to get her companions out, but she’ll not leave the room. This can be used to your advantage if you aren’t well equipped or leveled or farming with Hell difficulty.

Remember that the Loot Table of the Countess also works differently than regular enemies in dropping Runes. She has one particular Rune drop and then another bonus Rune drop.

Regular Rune Drop Table
Normal: El – Ral
Nightmare: El – Io
Hell: El – Ist
Bonus Rune Drop Table
Normal: El – Ral
Nightmare: El-Ko
Hell: El-Lo
The usual Rune drop tables are the ones she drops each time you kill her. It contains the most common Runes on the table. The bonus loot table generally includes the same Runes that the initial loot table has, but with the addition of four or two of the rarer ones. In the introduction of this guide, the Countess can drop as many as six runes in total. One of them is always from her usual loot table, and the others who fall are via her Bonus Table.
The last thing to mention is that if you’re playing on a single Player, I highly recommend keeping the game’s settings to one player, as this will increase the chances of obtaining the Rune from the Lady Countess’s loot table. If you increase the number of players playing, you’ll increase the number of Magical items that drop at the hands of The Countess. This can reduce the chances of finding additional Runes since the Magical thing that is dropped can be used in place of the Rune.
